Habilitation at Home (HAH)
This service is based out of the participants own residence. We provide outcome-based training to enhance personal skill level in the home and community. Designed to assist members in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in home and community-based settings, self-help/daily living. |
Attendant Care (ATC)
The Attendant Care Service is developed to attain or maintain safe and sanitary living conditions (personal and environmental), allowing an individual to remain in his/her personal home. Our objective is to develop and implement a schedule/general plan of care to ensure that the highest quality of life is achieved for the people that we serve. |
Respite Care (RSP)
Respite services is to provide member’s parents, caregivers and member’s representatives a short-term relief. Our agency is designed to provide quality care to individuals while offering a fun, safe, social opportunities in a familiar and nurturing environment. This service can be provided in individual's homes, various licensed community settings, group home, developmental home and nursing facilities. Center Based Employment (CBE)
Center Based Employment provides a controlled and protected work environment, additional supervision and other supports for individuals engaged in remunerative work either in a sheltered workshop or in the community. Our goal is to provide members with gainful, productive and paid work along with providing support to members in developing skills, abilities and behaviors that will enable them to most fully realize their vocational aspirations and support their transition into a more integrated employment setting. |
Transportation (TRA):
The agency will provide transportation to get members to and from day program, any pre-authorized medical appointment, work program, and all other activities pre-authorized by the agency (i.e. outing activities). Staff who transport members will be required to maintain a daily transportation log, have a valid Driver's License (audited monthly for current license status). |
Group Based Employment (GBE)
Our goal is to provide long-term, ongoing support services for an employed individual. This service will be provided at an onsite supervised work environment in a community employment setting. This service shall be provided in integrated community work settings, service shall be designed to promote community integration with other members of the workforce and provide paid work and transportation within the member’s scheduled workday from worksite to worksite shall be the responsibility of the agency. Agency’s goal is to provide members the opportunity to work in an environment that allows for maximum interaction among diverse populations, gainful, productive and paid work, support members in developing skills, abilities and behaviors that will enable most fully realize their vocational aspirations including supporting their transition into more independent employment setting, maintain positive work habits, attitudes, skills, work etiquette related to their specific employment, and assisting to become a part of the informal culture of workplace. |
Day treatment and training-adults (DTA)
DTA service that provides specialized sensory-motor, cognitive, communicative, social interaction and behavioral training to promote skill development. Designed to assist members in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in home and community-based settings, self-help/daily living. Our goal is to maximize current skills, initiate development of new skills and decrease or eliminate those behaviors that are most challenging and/or disruptive to a productive, community filled life. A monthly calendar of daily activities is developed with the member, members family and/or those representing members input included. Our Calendar is posted to see at the agency, agency websites and sent to the member, member’s representatives by e-mail or can be provided in-person. |
Adult programs are offered year-round, 8 hours a day (8am-4pm), Monday – Friday, except for the New Years, Memorial, Fourth of July, Labor, Thanksgiving and Christmas holidays.

All employees at Companion Day Center of Arizona meet the State of Arizona requirements, have a valid Fingerprint Clearance Card and have completed and passed the required training courses and background checks.
